The first appointment with a new patient is usually scheduled for 90 minutes and costs $450. To reduce the time and the fee, an online questionnaire can be provided which contains questions similar to those the doctor asks. The questionnaire is very thorough and has been carefully put together to ensure the doctor gains a full picture of each patient's problems. The form should take between 15 to 45 minutes to fill out depending on the complexity of the problems and health history (and your typing speed!).
To access the online questionnaire, a link with a secure login and password is provided by email when the first appointment is scheduled. Once the questionnaire is complete, pressing the “submit” button at the end of the form will automatically send it through to the doctor. A confirmation email will also be sent to the patient.
To take advantage of the time and fee reduction, patients must submit the questionnaire at least two business days prior to the appointment. The appointment is then reduced to 60 minutes and the fee to $300.
If a patient decides not to fill out the online questionnaire, then the doctor will still make time to go through everything in person. If a patient does complete the online questionnaire, it is important that the answers are as accurate and detailed as possible.
