A phone call, email or letter which sets out the following information would be appreciated:- The patient's concerns
- Your concerns
- Details of any medical conditions that may be relevant
- The type of treatment you have recommended
- Enclose any recent (<5 years) OPGs or intraoral films
- The extent you want our involvement (advise, advise and treat, take over management)
- The extent you want your involvement in future maintenance (all, shared, or none)
Our communication preference is for email (point form is fine), with Xrays or casts either posted separately or brought by the patient.
Please also let us know to what extent and how you would like us to report back to you about your patient. For example we generally provide brief summaries via email, phone or letter. Alternately full documentation can be provided if requested.
